Course Evaluation Request

Use this form to submit requests to have previously completed coursework evaluated for possible degree application.  Be aware that there are no guarantees of an evaluation approval. Submit the required documentation and await a determination from the Department. Once a course has been evaluated, it cannot be resubmitted again. Lower level courses (1000-2000 level) are unable to be substituted for advanced courses (3000-4000 level). Please allow up to five business days for processing. Do not also email the instructor of the class - submitting this form is the only process.

You will need to provide the following types of documentation:

  • Copy of Course Syllabus (Must be from the specific semester you took the course)
  • Course Description (from Institution's website)* This documentation is optional 
Must be a valid TXST Email address
(ex. A01234567)

(Format as XX ####, Ex. AG 1000)
